How Menstrual Cycle Length Affects Your Due Date: The Naegele Adjustment

The classic formula used to estimate a baby’s due date—known as Naegele’s Rule—was formulated by the German obstetrician Franz Karl Naegele in the early 19th century.

Naegele’s rule adds 1 year and 7 days to the first day of the last menstrual period, then subtracts 3 months (which is mathematically identical to adding 280 days).

However, Naegele’s rule makes one fundamental assumption: that every woman has a 28-day menstrual cycle with ovulation occurring on Day 14.

The Reality of Cycle Length Variations

In clinical reality, normal menstrual cycles range anywhere from 21 to 45 days or longer.

The human menstrual cycle consists of two distinct phases:

  1. The Follicular Phase: The time from the first day of bleeding until ovulation. This phase varies widely between individuals.
  2. The Luteal Phase: The time from ovulation until the next period. This phase is remarkably constant across most individuals at 14 days (±1 day).

The Mathematical Formula

Because the luteal phase remains ~14 days, the day of ovulation is determined by:

Ovulation Day = Cycle Length - 14

Why Gestational Age Must Be Adjusted

If a pregnant individual with a 45-day cycle uses an unadjusted LMP calculator, the calculator will overestimate gestational age by 17 full days.

This error could lead to:

  • False alarm about slow embryonic growth on early ultrasounds.
  • Unnecessary early labor induction due to an artificially inflated due date.
